thank you for choosing Cosmodent for your dental treatments. Before
you come to Cosmodent for your first appointment, we ask you to read
over the following information carefully. This information will ensure
that we will have a long lasting and successful relationship.
Things we need to know about you

|
When did you last visit a dentist? If you have
x-rays taken at this appointment, please bring them with you.
|
|
|
Do you suffer from any systemic disease
(cardiac disorder, high blood pressure, diabetes, etc..), and
do you regularly take any medication (aspirin, etc) |
|
|
If you have a systemic disease , please bring the
name and the telephone number of your doctor who treats this
disease. |
|
|
If you have a special conditions nowadays (like
pregnancy), please inform us. |

|
If you have dental pain |

- When did you first have this pain, and how long did it
last ?
- What are the causes of pain? (cold, hot, sweet, pressure)
- Did any part of your face become swollen ?
- Did you take any medication? Are you still taking it
? Please write down the names for us.

|
What are your expectations from us ? If you have
some personal aesthetic expectations , please describe this
to us in writing. |

appointments
|

|
|
Mrs. Talin Kibar is "appointment
coordinator" at Cosmodent. She speaks French and English.
|
|
|
We know that time is precious to everybody.
For that reason, we prefer to accept patients "by appointment." |
|
|
To run the daily program punctually, it will be
greatly appreciated if you show up 5-10 minutes before your
appointment time. |
|
|
For each appointment there will be a certain time
reserved for you. If you are late because of unexpected circumstances,
another procedure that can be completed in the "time left"
may be chosen for the day. |
|
|
If you are unable to come for the appointment
taken, you should inform the "appointment coordinator"
at the "latest 24 hours"; in advance. |
|
|
There are specified car parks nearby
as shown on the "location " page. If you wish to use
the "valet parking" service provided for Cosmodent
guests, you should inform the receptionist in advance.
Our driver will take your car from you in front of the building,
and will park it for you. At the end of your appointment, your
car will be brought back to the same location.

 Financial
arrangements
|

Our fees are based on
the quality of the materials we use and our experience in performing
your needed treatment. |
|
|
Mrs. Azra Ucak is
financial coordinator at Cosmodent. Please do not hesitate to
contact her if you ever feel uncomfortable with financial matters. |
|
|
During your first appointment, all
your teeth and mouth will be examined. To keep your initial
status for our records , a panoramic x-ray will be taken. (if
you have one , please bring it with you). Your problems will
be diagnosed, and a treatment plan will be determined in cooperation
with you. |
|
|
If you wish to learn, the estimated cost of the
treatment, it will be given to you . Please feel free to discuss
the payment options together with the financial coordinator.

|
Treatments started and completed in one appointment
(fillings, extractions, cleaning, etc.) are expected to be paid
for on the same day. |
|
|
 Treatments
like "comprehensive dental rehabilitation" or "full
mouth implant rehabilitation" will have relatively high
budgets and each case will have unique circumstances. For
this reason , it is more rational to discuss the payment plans
for such cases individually. Our financial coordinator will
be happy to discuss each case on a one to one basis.
We believe that the agreement on the treatment plan and its
estimated cost, made before the treatment starts, is a critical
factor in having a long lasting ,and healthy relationship.
- At your first visit a "dental examination" fee
is applicable.
- Payments may be made either in cash or by credit card, Visa
and Master Card are welcome.

We believe that in choosing your dentist price level should
not be the prime criteria. Every single case has its own character
and should be considered individually. For this purpose, in
this web page, we prefer not to announce a price list and
do not answer questions considering the prices.
